clearly the person who designed missed punching shear check. , if pinching shear check was done thn this issue could have been cured with increase in slab thickness, adding drop on slab and other solution {{1629751732}} mentioned.

Punching shear can be avoided if proper reinforcement can be provided. column capital is normally provided where PT SLAB is taking place.

I agree with what {{1629751732}} has said here. but there can be one more reason. releasing the shoring(supports for formwork) prematurely can also lead to this failure. In this situation loads from two floors are acting on that slab. what my point is, this can be a workmanship issue also.
